Dr. Thomas Schweller, MD, is triple board certified neurologist in Neurology & Psychiatry, Pediatrics and Electroencephalogram (EEG). He is also a qualified medical evaluator (QME), independent medical examiner (IME) and agreed medical examiner (AME).

Dr. Schweller received his Doctor of Medicine from Georgetown University in Washington, D.C., and completed is pediatric residency at the Children’s Hospital of Philadelphia, and his fellowship in Neurology at the Mayo Clinic in Rochester, Minnesota.

In 1988, Dr. Schweller began his private practice in San Diego which has consisted of private patients, AME’s, QME’s and independent medical examinations for the Department of Social Services, Social Security in neurology and pediatrics. Dr. Kevin Yoo is one of the premier neuro-spine specialists in Southern California. He received his bachelor’s degree from University of North Carolina at Chapel Hill as a Morehead Scholar. He went onto Emory University School of Medicine where he became a doctor in medicine and did his Internship. He moved onto the University of Arkansas for Medical Sciences where he attended graduate school and did two years of research on brain tumors. Dr. Ward is a nationally recognized expert in brain function and how that affects mood, work, and behavior. She was born in Washington DC but quickly moved throughout the world, eventually landing and being raised in California from childhood. Academically, she earned a BS from California Polytechnic State University, SLO; an MEd from City University, Bellevue, WA; and both an MA and PsyD in Clinical Psychology from the American School of Professional Psychology/Argosy, Washington DC. Additionally, she went on to complete a two-year Houston Guideline fellowship in neuropsychology with an additional focus on forensics.

In 2010, Dr. Ward began her private practice in the Virginia / Washington DC area. There, she worked on private cases and consulted at Snowden Community Psychiatric Hospital, Spotyslvania Hospital, Mary Washington Medical Center, HealthSouth Rehabilitation Hospital, and numerous assisted living and skilled nursing facilities. She also was frequently contracted with multiple county courts for forensic evaluations as well as multiple workman’s compensation agencies. With that experience, she became the Clinical and Training Director at Neuropsychology & Complementary Medicine and Central Coast Neuropsychology, where her team created a psychology/neuropsychology training program including externships, doctoral practicum, pre-doctoral clinical psychology internships, and two-year Houston Guideline approved post-doctoral fellowships in neuropsychology. Dr. Amir Bahreman, MD, is a quadruple board-certified neurologist, and is an American Diplomat in Neurology and Psychiatry, Brain Injury Medicine, Clinical Neurophysiology and Vascular Neurology by APBN.

Dr. Bahreman trained in neurology at the Albert Einstein University in New York in 2000. During his residency at Montefiore Hospital he developed additional expertise in headache disorders, stroke and neuromuscular diseases. Because of the unique location of Montefiore Hospital in the Bronx New York, he was exposed to a variety of neurological pathologies including traumatic brain injury. During his residency he treated patients with traumatic brain injury, spine injury and neuromuscular disorders. In his last year of residency, he had the privilege of working directly with Dr. Richard Lipton and Dr. Lawrence Newman, world renowned headache experts, which gave him the knowledge necessary to diagnose and manage post traumatic headache disorders and migraines associated with traumatic brain injury.

Following residency, Dr. Bahreman, entered a fellowship program at the University Of Chicago, where he became board certified in Clinical Neurophysiology in 2007 and Vascular Neurology (stroke) in 2008. During his fellowship training at University of Chicago he became skilled in electrodiagnostics including electroencephalography, electromyography, and nerve conduction studies.

Upon completing his fellowship, Dr. Bahreman joined a local private neurology group and in 2008, he moved to San Diego when he started his own solo practice. He became affiliate with Sharp Health System where he presently provide services as Stroke specialist at Sharp Grossmont Hospital. In his private practice, he treats patients with traumatic brain injury, post traumatic headache disorders, migraines, complex regional pain syndrome (CRPS), neuralgias, seizures, musculoskeletal diseases, Parkinson’s disease, stroke, spinal cord injuries, traumatic spinal disease and peripheral nerve injuries.
Since then, he has received extra training and become certified in a variety of areas including VNG (video nystagmography), posturography, Botox and spasticity, and antiaging medicine. ABOUT THE INPATIENT REHABILITATION FACILITY/ SKILLED NURSING SERVICES/ ACUTE SUBACUTE

Our Mission:

We are committed to providing the highest quality of subacute care, post-acute care and rehabilitation therapies to individuals who are recovering from catastrophic injury events in personal injury and workers compensation. We accomplish this goal by a staffing patient ratio of 2 fulltime staff members per patient, providing our full attention to the plan of care that is individualized per patient to ensure their progression to wellness. Larger facilities house many patients with differing infections and diseases. We limit our patient load to only 6 at a time therefore reducing the risk of acquired infections as well as adhering to our strict infection control protocols.

Who We Are:

Managed by a multidisciplinary team of physicians, nurses and therapists, Coastal Injury Care Center is an inpatient rehabilitation facility and subacute care center that provides round-the-clock skilled nursing services and intensive rehabilitation therapies to individuals who have been severely and catastrophically injured. Our individualized care plans are managed by an interdisciplinary group of specialists including neurologists, orthopedists, physiatrists, podiatrists, wound care physicians, dentists, nurses, social workers, activity directors, respiratory therapists, registered dieticians, physical therapists, occupational therapists and speech therapists.

Common Injuries:
Coastal Injury Care Center provides services based upon each resident’s initial assessment, including but not limited to:
· Acquired Brain Injuries (traumatic, anoxic, aneurysms, cerebral vascular accidents)
· Spinal Cord Injuries (all levels including Halo care)
· Complex Wound Care (including V.A.C. Therapy)
· Pulmonary (Vent care and weaning, tracheostomy care, suctioning and de-cannulation)
· Amputee Care
· Medically Complex (multiple IV antibiotic therapy, isolation, TPN, G and J tube feeding)
· High-Intensity Rehabilitation (PT, OT, ST, LCSW, MSW)
· Orthopedically Complex (multiple trauma, fractures, non-weight bearing)

Services:
Coastal Injury Care Center provides the following services for patients in need of acute and subacute care.
· Individualized treatment programs managed by a multidisciplinary group of physicians including neurologists, orthopedists, physiatrists, podiatrists and wound care physicians.
· Continuous (24-hour) skilled nursing and supportive care provided by licensed healthcare professionals under the supervision and direction of a board certified Medical Director (MD/DO).
· Neurologic and/or vascular monitoring
· Ventilator or tracheostomy dependence/complex pulmonary care
· Respiratory Therapy services for oxygen delivery, suctioning, CPAP or BiPAP.
· Dietary care including tube feeding or total parenteral nutrition (TPN)
· Gastrostomy or ileostomy care not manageable at a lower level of care.
· Complex wound care (debridement, packing, extensive dressing)
· Medication monitoring
· Psychosocial support provided by licensed clinical social workers and psychiatrists.
· Subacute rehabilitation for physical, occupational and recreational therapy provided by licensed physical and occupational therapists.
· Skin care, oral care and position changes

Located 15 minutes outside of downtown San Diego in La Mesa, our newly remodeled facility has a fully equipped kitchen, entertainment area, rehabilitation room for therapeutic activities, a fully landscaped backyard and spacious rooms. Jason Morris, DPM

Board Certified in Foot & Ankle Surgery

After completing a comprehensive three-year residency at the University of Pittsburgh, Jason Morris, DPM, moved to California to pursue a career in podiatric medicine. Following his move, Dr. Morris worked as an attending physician at UCLA Medical Center and Cedars Sinai Hospital in Los Angeles. Since relocating to the Escondido area, he has been a staff physician at Palomar Medical Center in Escondido and Poway. He is a podiatric foot and ankle specialist with board certification in rearfoot and forefoot reconstructive surgery. Dr. Morris has undergone extensive training in sports medicine, reconstructive surgery, foot and ankle trauma, and diabetic limb salvage.
